Bowtie analysis is one of the most widely adopted risk assessment tools in high-hazard industry. It is recognized by regulators, expected by major auditors, and referenced across oil and gas, chemical, and petrochemical standards. Yet the quality of Bowtie studies varies enormously, and the gap almost always comes down to the same root cause: the people developing the diagrams have not been trained to do it properly.
Bowtie is not difficult to learn, but it is easy to do badly. Threats get confused with hazards. Barriers become vague activities rather than specific, testable controls.
